ارزيابي خطاهاي انساني به روش‏ تجزيه و تحليل استانداردشده صنعتي ريسك در پرستاران بخش اورژانس زايمان يكي از بيمارستان هاي دانشگاه علوم پزشكي تهران

Evaluation of human errors, using Standardized Plant Analysis Risk Human Reliability Analysis technique, among nurses of delivery emergency ward of one of the Tehran University of Medical Sciences hospitals

زمينه و هدف: اخيرا موضوع خطاهای پزشكی در سيستم درمانی مورد توجه قرار گرفته است. با در نظر گرفتن اين موضوع كه پرستاران نسبت به ديگر كاركنان، زمان طولانی‌تری را با بيماران سپری می‏كنند بيش از بقيه كاركنان مستعد انجام خطای انسانی می‌باشند. در مطالعه حاضر خطاهای انسانی پرستاران بخش اورژانس زايمان مشغول به كار در يكی از بيمارستان‌های دانشگاه علوم پزشكی تهران مورد بررسی قرار گرفت. مواد و روش‌ها: در مطالعه مقطعی حاضر، ابتدا تجزيه‏ و تحليل وظايف حرفه پرستاری بخش اورژانس زايمان با تكنيك تجزيه و تحليل سلسله مراتبی (HTA) با تشكيل Focus Group انجام شد. در مرحله بعد، احتمال خطای انسانی با روشSPAR-H تعيين و هر يك از فاكتورهای شكل دهی عملكرد در برگه‏كار‏ها و فعاليت‏های تشخيصی و عملی مشخص ‏گرديد. پس از تعيين مقدار وابستگی، ميزان احتمال نهايی خطا محاسبه ‏گرديده و با استفاده از جداول احتمال خطا و شدت خطا، ارزيابی سطح ريسك انجام شد. نتايج: طبق نتايج، در مورد چهار گروه وظايف، كمترين خطا مربوط به زير وظيفه‌ی " كار با دستگاه پمپ سرم" با ميزان خطای 055/0 و هم چنين بيشترين احتمال خطا مربوط به زير وظيفه ی" تزريق فرآورده های خونی" با ميزان احتمال 780/0 برآورد گرديد. همچنين، مسؤليت‌های خاص بخش از بالاترين ريسك های نامطلوب برخوردار بودند. نتيجه گيری: تحليل خطاهای شناسايی شده در مطالعه حاضر نشان می‌دهد كه بدليل حجم بالای كار، زمان ناكافی و همچنين نياز به رسيدگی مديريتی بسيار دقيق و برقراری هماهنگی های لازم، علل عمده خطاها را می‌توان به استرس و پيچيدگی بالا در وظايف اين بخش نسبت داد.

Background: Considerable attention has been paid recently to medical errors in health care system. Taking into account that nurses spend more time with patients in comparison with other staff, they are more prone to human errors. The present research was done to investigate nurses' errors in delivery emergency ward of one of the Tehran University of Medical Sciences hospitals. Material & Methods: In the present coss-sectional study, first, task analysis was conducted for nursing proffession, using Hierarchical task analysis (HTA) technique. Following, the Human Error Probability (HEP) was determined employing SPAR-H method and dependences of performance-shaping factors (PSFs) and action and diagnosis activities were identified. Finally, after determinig the level of dependency, the Final Diagnosis HEP was calculated. Assessment of risk level was done using the risks' probability and intensity tables. Results: According to the results, for the four groups of understudy tasks, the lowest errors were belonged to the subtask "working with serum pumping machine'' with the error level of 0.055 and the highest error probability related to the subtask '' transfusion of blood products'' with the error rate of 0.78. Moreover, special responsibilities of the wards had the highest level of undesirable risks. Conclusion: The analysis of identified errors in the present study shows that due to high work demand, insufficient time, and the need for accurate administrative monitoring and providing required arrangements, the main causes of errors can be attributed to high level of stress and complexity in the tasks of this ward.
