رويكرد اخلاقي در ارائه‌ي خدمات طبي، حمايتي و تسكيني به نوزادان مبتلا به نارسايي مزمن كليه و خانواده‌هاي آنان

An ethical approach to providing medical, supportive and palliative services for infants with chronic renal failure and their families

نارسایی مزمن كلیوی وضعیتی تهدیدگر برای زندگی نوزادان است كه خانواده­های آنان را نیز تهدید می­كند. بیماران مبتلا به نارسایی مزمن كلیه و خانواده­های آنان تحت فشارهای جسمی، روانی و اجتماعی زیادی قرار می­گیرند. بنابراین، به خدمات تسكینی و حمایتی نیازمند می­شوند. به­علاوه، اخلاق نقش مهمی در مراقبت از این بیماران و خانواده­های آنان دارد. هدف این مطالعه، شناسایی رویكردی اخلاقی در زمینه‌ی فراهم­سازی خدمات طبی، تسكینی و حمایتی برای نوزادان مبتلا به نارسایی مزمن كلیه و خانواده­های آنان است. مطالعه‌ی حاضر، بر مبنای یك نمونه در گراند راند اخلاق پزشكی مركز طبی كودكان تهران انجام شده‌است. نمونه‌ی معرفی شده، یك نوزاد پسر با تشخیص نارسایی كلیه بود كه در ارزیابی­های انجام شده، هیدرونفروز دوطرفه‌ی شدید و كیست­های متعدد كلیوی، همراه با كراتینین بالا یافت شد. در بررسی­های بعدی هم­چنان فیلتراسیون گلومرولی پایین و فشارخون بالا وجود داشت. پزشكان پس از انجام دیالیز صفاقی اولیه، تعبیه كاتتر دیالیز صفاقی مداوم سرپایی (Continuous ambulatory peritoneal dialysis: CAPD) را تحت عمل جراحی برای انجام دیالیز مستمر پیشنهاد دادند تا در صورت رسیدن شیرخوار به سن بالاتر، كاندید پیوند كلیه شود. اما والدین رضایت ندادند و بیمار را با رضایت شخصی ترخیص كردند كه چند هفته بعد، نوزاد در منزل فوت كرد. به‌نظر می­رسد كاركنان درمانی نیازمند به­كارگیری راهبردهای كارآمد اخلاقی جهت ارائه‌ی خدمات طبی، حمایتی و تسكینی به نوزادان مبتلا به نارسایی مزمن كلیه و خانواده­های آنان هستند. این خدمات، علاوه بر خدمات تسكینی و مراقبتی جهت خانواده و بیمار، شامل حمایت روانی و یاری به خانواده‌ی بیمار در اتخاذ تصمیمی می­شود كه كم‌ترین ضرر را عاید بیمار ­كند.

Chronic renal failure in infants is a life-threatening condition that can also severely affect their families. Patients and their families are under great physical, mental and social pressure, and therefore require medical, palliative and supportive care. Moreover, ethics has an important role in care for these infants and their families. The purpose of this study was to identify an ethical approach to providing medical, supportive and palliative services for infants with chronic renal failure and their families. The study was based on a case report in the Medical Ethics Grand Rounds of the Childrenʹs Medical Center in Tehran, Iran. The case pertained to a male newborn infant with diagnosis of renal failure. Assessments indicated severe bilateral hydronephrosis and multicystic dysplastic kidneys accompanied by high creatinine levels. The patient underwent surgery but further evaluations revealed a low glomerular filtration rate accompanied by elevated blood pressure. The physicians suggested insertion of a catheter for CAPD (continuous ambulatory peritoneal dialysis) after the initial peritoneal dialysis until a kidney transplant was performed. The proposal, however, was rejected by the parents and the patient was discharged by their consent. A few weeks later, the infant expired at home.
