مقايسه نتايج ماموپلاستي به روش پديكول مياني با ماموپلاستي به روش پديكول تحتاني

Comparing medial pedicle mammoplasty with inferior pedicle mammoplasty

زمينه و هدف عمل جراحي كاهنده پستان يكي از شايع ترين اعمال جراحي زيبايي است. تا كنون روش هاي بسياري براي كاهش پستان توضيح داده شده اند از جمله روش هاي پديكول مياني و پديكول تحتاني. هدف از انجام اين مطالعه مقايسه نتايج ماموپلاستي به روش پديكول مياني و مقايسه آن با ماموپلاستي به روش پديكول تحتاني است. روش كار 33 بيمار متوالي كه در بيمارستان 15 خرداد تحت عمل جراحي ماموپلاستي به روش پديكول مياني قرار گرفته بودند، با 33 بيمار كه به روش پديكول تحتاني ماموپلاستي شده بودند، مقايسه شدند. سن بيماران، مقدار نسج برداشته شده از پستان ها، طول مدت عمل جراحي، ميانگين فاصله استرنال ناچ تا نيپل ها، عوارض شامل Bottoming out، نياز به انجام عمل جراحي دوباره، درخواست برداشتن اسكار محل عمل، كنتور پستان، تقارن بين دو پستان، حس گيري نيپل و آرئول، ميزان وقوع عوارض، فراواني از دست رفتن نيپلآرئول و ميزان رضايت بيماران از طول اسكار و رضايتمندي كلي بيماران بررسي شد. يافته ها ميانگين سن در دو گروه مشابه بود. در روش پديكول مياني طول مدت عمل جراحي (20. 16 ±177. 73 دقيقه در برابر 26. 77 ±221. 06 دقيقه، P=0. 01) و مقدار نسج برداشته شده از پستان ها (به ترتيب طرف چپ 91. 57 ± 454. 24 گرم در برابر 401. 28 ±900. 91 گرم و در طرف راست 82. 87 ± 446. 67 گرم در برابر 407. 88 ± 904. 24 گرم، P=0. 01) و ميانگين فاصله استرنال ناچ تا نيپل ها (3. 37 ± 29. 09 در برابر 3. 36 ± 35. 33 سانتيمتر، P=0. 01) كه به طور معناداري از روش پديكول تحتاني كمتر بودند. اين در حالي است كه فراواني Bottoming out در اين گروه پديكول تحتاني (30. 3 درصد در برابر 0 درصد، P=0. 01) و رضايت بيماران از طول اسكار كمتر بود (54. 5 درصد در برابر 87. 9 درصد، P=0. 003). تفاوت معناداري بين نياز به انجام عمل جراحي دوباره، درخواست برداشتن اسكار محل عمل، كنتور پستان، تقارن بين دو پستان، حس گيري نيپل و آرئول، ميزان وقوع عوارض، فراواني از دست رفتن نيپلآرئول و ميزان رضايتمندي بيماران وجود نداشت. نتيجه گيري و توصيه ها نتايج مطالعه حاضر نشان داد كه در ماموپلاستي به روش پديكول مياني اگرچه مقدار نسج برداشته شده از پستان ها كمتر است، ولي طول مدت عمل جراحي كوتاه تر است و با نتايج زيبايي بهتر و رضايت بيشتري همراه است.

Background and Aim: Breast reduction is one of the most commonly performed plastic surgery procedures. Many approaches to breast reduction have been described, including medial pedicle and inferior pedicle. The aim of this study was to compare the results of medial pedicle mammoplasty method with inferior pedicle mammoplasty. Methods: Thirtythree consecutive patients who had undergone medial pedicle mammoplasty at 15 khordad hospital were compared with 33 inferior pedicle mammoplasty. Patients age, breast resection weight, surgery length, the average distance from the sternal notch to the breast nipples, complication rate including bottoming out, need for reoperation, request for surgical site scar removal, breast contour, breast symmetry, nipple and areolar sensation, the frequency of complications, nippleareolar complex necrosis and patientsrsquo; satisfaction from scar length and overall patientsrsquo; satisfaction were assessed. Results: Mean age was comparable in both groups. In medial pedicle, the surgery length (177.73plusmn;20.16 min vs. 221.06plusmn;26.77, P=0.01) and resected breast weight (respectively, 454.24plusmn;91.57 vs. 900.91plusmn;401.28 gr in the right side and 446.67plusmn;82.87 vs. 904.24plusmn;407.88 gr in the left side) and the average distance from the sternal notch to the breast nipples (29.09plusmn;3.37 vs. 35.33plusmn;3.36 cm, P=0.01) were significantly lower compared to inferior pedicle group. Yet, the frequency of bottoming out was significantly higher (30.3% vs. 0%, p=0.01) and the patients satisfaction from scar length was lower (54.5% vs. 87.9%, p=0. 003) in the medial pedicle group. There were no significant difference between the need for reoperation, request for surgical site scar removal, breast contour, breast symmetry, nipple and areolar sensation, the frequency of complications, nippleareolar complex necrosis and patientsrsquo; satisfaction. Conclusions: The results of the present study showed that, although the resected breast weight was lower in medial pedicle mammoplasty, the length of surgery was shorter and better results regarding aesthetic point of view and higher patientsrsquo; satisfaction are achieved.nbsp;
